Section 1540.70  Death Benefits

a)         Benefit Application

            Any person claiming a death benefit, shall file at the Springfield Office of the System a written application for such benefit on a form prescribed by the Board.

b)         Proof of Death

            An application for a death benefit shall be accompanied by a certified copy of the Certificate of Death.

c)         Identification of Claimant

            Any person or persons applying for death benefits of any kind shall, when requested by the Board, file at the Springfield Office of the System an affidavit alleging at least the name of the claimant and his or her relationship to the deceased member.
