Section 1460.50  Restoration

Applications for restoration of a limited or unlimited roofing license shall be made to the Department on forms provided by the Department and shall include:

a)         Designation as either a limited or unlimited roofing contractor;

b)         Proof of insurance as required in Section 1460.20;

c)         Proof of continuous surety bond as required in Section 1460.30;

d)         The name of the designated qualifying party who has passed the examination set forth in Section 1460.12.  The Department shall not require an individual whose license has lapsed less than 90 days to sit for the examination; and

e)         The required fee set forth in Section 1460.80.
