Section 1230.60  Application for Licensure Examination

An applicant for licensure shall submit an application at least 30 days prior to an examination date.  The application shall include a certified transcript of registered training hours the applicant has completed, a recent photograph not larger than 2½ by 2½ inches, and the fee required by Section 1230.155.
