SECTION 350.410 REPORTING FATALITIES, HOSPITALIZATIONS, AMPUTATIONS AND LOSS OF EYE INCIDENTS TO THE ILLINOIS DEPARTMENT OF LABOR
Within 8 hours after the death of any employee from a work-related incident, the employer shall orally report the fatality by telephone 24/7 Notification – (800) 782-7860 or (217) 782-7860. Within 24 hours after the in-patient hospitalization of one or more employees, or an employee's amputation, or an employee's loss of an eye, as a result of a work-related incident, the employer shall report the in‑patient hospitalization, amputation or loss of an eye.
1) The reporter must give the following information for each fatality or hospitalization incident:
A) The establishment name;
5) If the employer does not learn of a reportable incident at the time it occurs and the incident would otherwise be reportable under this Section, make the report within 8 hours after the incident is reported to the employer or any agent or employee of the employer.
