Section 392.40  Form of Request for a Hearing

The Request for a Hearing shall be filed within 10 days after receipt of an administrative decision and shall:

a)         clearly state the name of the respondent;

b)         identify the administrative decision with respect to which a hearing is requested;

c)         be typewritten on 8½ x 11 inch paper;

d)         be signed by the respondent or by the respondent's attorney and shall contain the address and telephone number of the individual signing the document; and

e)         be sent by either certified mail, return receipt requested, or by personal delivery to the Office of Banks and Real Estate, 500 East Monroe, Springfield, Illinois  62701.
