Police and Fire Pension Fund Study

 

Greetings:

 

Thank you for taking the time to complete this web-based questionnaire on the financial condition of your local police and fire pension funds. The link to the Pension Fund Questionnaire is located below. This information is being sought pursuant to P.A. 96-1495, which requires the Commission on Government Forecasting and Accountability to complete a study on the financial condition of all Article 3 (police) and Article 4 (fire) pension funds governed under the Illinois Pension Code. On our website, you will find links to similar reports that the Commission has completed pertaining to the State-funded retirement systems, as well as the Chicago, Cook County, and IMRF systems. In compiling this information, we hope to produce a report on police and fire pension funds that is similar in scope to the aforementioned financial condition reports. Please submit the requested information no later than November 4, 2011.

Form Instructions:

 

  1. If the fields are not highlighted upon opening, click on the "Highlight fields" box located on the right at the top of the form if you would like to see the required fields to fill out.
  2. Tab to get to the next field or mouse click to move between the fields.
  3. The form does not allow $ or % symbols, so please enter numeric values only where required.

 

Submission Instructions: Depending on your computer's settings, one of two screens will popup upon clicking the Submit button. If the popup is named "Select Email Client" you will have the following choices:

 

  1. "Desktop Email Application" - If you have an email program installed on your computer, such as MS Outlook or Apple's Mail, choose this option. It will automatically open that program with the correct email address to send to and the survey information attached in the required format. Then just click send in your email program.

 

  1. If you do not have an email program installed on your computer, choose "Internet Email". This will allow you to save the file as an .fdf file somewhere on your computer. Then, open your Internet email program and attach the .fdf file. Please email to PensionStudy@ilga.gov.

 

The second possible pop up would be named "Send Data File" and will have the following choices:

 

1.      "Send Data File" - which will try to open up an email program installed on your computer.

 

2.      "Cancel". If there is no email program installed on your computer, then you will have to go to the Adobe controls within the form window and save the file, then email it to PensionStudy@ilga.gov.

 

3.      "Print Form". The final option is to print the form and mail it to:

NOTE: Completed fields will not clear upon exiting so after Submitting and Printing out the form for your records, clear the form if you do not want the information to remain on the web form.
